New Patient Request Form

Dr. Leeann is currently accepting new IHN and BCBS patients for teletherapy. Dr. Leeann provides individual therapy for adults (age 18 and older). Dr. Leeann does not provide couples therapy.

Dr. Leeann is an in-network provider with Oregon Health Plan (OHP), InterCommunity Heath Network (IHN), and Blue Cross/Blue Shield - Regence (BCBS).
